Understanding the changes that occur during the dying process is complex, and Hospice & Community Care wants to help your team walk this journey. Referring your resident to Hospice & Community Care early in their illness will give your resident and their family the opportunity to share their goals to enhance their quality of life, as well as their comfort measures. Hospice & Community Care often hears from patients and their families that they wish they would have sought end-of-life care earlier in their diagnosis.

The following are general physical and behavioral indicators that your resident is ready for hospice care.

  • Increased restlessness
  • Withdrawing lack of engagement
  • Increased sleep
  • Decreased intake
  • Pauses in breathing
  • Settling unfinished business
  • New incontinence bowel and bladder
  • Increased swelling

The following questions may help determine whether your resident needs our help. Contact Hospice & Community Care’s Admissions Team at (844) 422-4031 if your resident:

  • May not survive within the year
  • Is seriously ill and pain management is needed
  • Has more than one chronic condition
  • Has been admitted to the ER/hospital several times within six months
  • Is facing difficult conversations regarding end-of-life decision
