Description
Hospice & Community Care’s York Ambassador Council invites you and a guest to join them for an evening of socializing and a short program by Dr. Colleen Hazlett O’Brien, Medical Director of In-Home Palliative Care, Hospice & Community Care. Dr. Hazlett O’Brien will talk about the organization’s new Choices Palliative & Supportive Care at Home program. The Choices program is specialized home-based medical care for people living with any serious illness.
There will be an open bar, featuring Thorn Hill Vineyards wine, and hors d’oeuvres.
