What We Offer

Hospice & Palliative Care Essentials

  • Hospice Care 101 – The hospice philosophy guides the development for individualized, comfort plans of care. This program explores this philosophy as well as eligibility requirements, pain and symptom management, when to refer a patient or resident, and the benefits of early referral.
  • Palliative Care 101 – Palliative care provides an additional layer of support for patients and residents who are living with serious illness.  This program explains the basics of palliative care, including eligibility requirements and the process for making a referral.
  • Hospice Care for the Hospitalized Patient – Developed for hospital staff, this session differentiates between the different levels of hospice care and the explains the criteria for each type. General Inpatient Level of Care in the hospital setting is discussed.
  • Approaching Life’s End – What to Expect – What changes occur as individuals progress through the natural process of dying? This session explains what to expect in the last months, weeks, days, and hours of life. Strategies for working with hospice care teams are also discussed.

Bereavement Support Training

Grief, Loss & Bereavement – Hospice & Community Care’s Pathways Center for Grief & Loss provides consultation and trainings to employers, social service and healthcare workers, church communities, and others. Dementia Care & Validation Techniques

  • Dementia Programs – Hospice & Community Care offers a wide variety of educational programs focused on different aspects of dementia to help support individuals and their loved ones. Topics range from effective communication strategies, responding to dementia-related behaviors, and considerations for care in later stages of the disease. Contact us to discuss the development of a program that fits the unique interests and needs of your group.
  • Compassionate Care for People Living with Dementia – There are many ways palliative and hospice care can provide comfort for people living with dementia and their loved ones. This program provides an overview of both types of care and discusses when to consider making a referral.
  • Dementia Friends Information Session – This program was designed to raise awareness, reduce stigma, and increase support for those living with dementia in our community. Five key messages about dementia and tips for interactions are discussed. Participants are invited to become a Dementia Friend at the end of the session by committing to one small action that will improve the lives of people living with dementia.

  • Is It Time to Consider a Referral? – When is the right time to consider hospice care? This program explains eligibility requirements and triggers, the process for making a referral, and tips for having conversations about end-of-life care.
  • Working with Hospice Teams – This session explores what to expect as end of life nears and discusses tips for working with hospice care teams to provide comfort for patients and residents.

Pain & Symptom Management

  • End-of-Life Hydration and Nutrition – Changes in hydration and nutrition needs at end of life can be challenging to understand and address. This program explains the changes that occur during the natural dying process. Common concerns and essential care needs are also discussed.
  • Pain and Symptom Management for Senior Living Residents – Pain and symptoms that are often experienced as people near end of life impact the quality of life in many ways. This program explains how hospice teams manage pain and symptoms based on the unique needs of each individual.

Ethical & Legal Considerations

End-of-Life Care Conversations – This program explores barriers to effective communication and provides tips for having challenging conversations with patients or residents at end of life.

Self-Care for Healthcare Professionals

  • Self-Care for Senior Living Professionals – Caring for patients or residents who are living with serious illness is both rewarding and challenging. This program, designed specifically for Senior Living Professionals, discusses common signs of stress and compassion fatigue and provides tips for self-care
  • Anchor Your Heart Grounding Tool – This session, developed by the non-profit organization Opus Peace, introduces participants to the Anchor Your Heart strategy.  This self-grounding tool can be used in times when strength, courage, or patience is needed.
