What We Offer
Hospice & Palliative Care
- Hospice Care: Myths and Facts – There are many myths surrounding hospice care. This program explores misconceptions and identifies how selecting this type of care can improve quality of life for the patient, while also supporting loved ones.
- Palliative Care: Myths and Facts – Palliative care is often confused with hospice care. Differences between these types of care are explained, and the benefits palliative care for anyone living with serious illness is discussed.
- HOPES Trivia – Hospice Offers People Extra Support -This interactive session reveals hospice care through a trivia format. Attendees will learn how hospice care supports patients, loved ones, and caregivers through serious illness and end of life.
How to Support Your Loved One
Supporting Loved Ones at End of Life – It is hard to know what to do or say when a loved one is nearing end of life. This program is designed for people who want to learn more about how to support and communicate with people during this time. What to expect, considerations for visiting, and suggestions for supporting friends or loved ones are discussed.
Dementia Care & Validation Techniques
- Dementia Friends Information Session – This program was designed to raise awareness, reduce stigma, and increase support for those living with dementia in our community. Five key messages about dementia and tips for interactions are discussed. Participants are invited to become a Dementia Friend at the end of the session by committing to one small action that will improve the lives of people living with dementia.
- Dementia Programs – Hospice & Community Care offers a wide variety of educational programs focused on different aspects of dementia to help support individuals and their loved ones through the dementia journey. Topics range from understanding dementia, effective communication, responding to dementia-related behaviors, and considerations for care in later stages of the disease. - Compassionate Care for People Living with Dementia – There are many ways palliative and hospice care provides comfort for people living with dementia and their loved ones. This session shares an overview of both types of care and discusses when to consider making a referral.

Self-Care for Caregivers – Caring for a loved one who is living with serious illness can be both rewarding and challenging. This program stresses the importance of maintaining your own well-being while caregiving. Strategies and tips for self-care are discussed.
Making Your Wishes Known
Making Your Wishes Known – Do you know what kind of care you would want in a medical crisis? Have you discussed your wishes with loved ones? This program explains the reasons planning for future health care is so important and explores steps and tools for completing the process.
What You Can Expect from Hospice
What Can I Expect from Hospice? – This program covers the hospice philosophy, eligibility and when to consider making a referral, hospice teams, services, and how to get more information.
Grief, Loss & the Bereavement Process
Grief, Loss & the Bereavement Process – Hospice & Community Care’s Pathways Center for Grief & Loss provides consultation and trainings to employers, social service and healthcare workers, church communities, and others.
